Plus Plan
There’s no coverage outside the BSMH Plus Network. Use BSMH Plus Plan directory only.

HealthSpan Partners Extended Plan
Use the Extended Plan’s Network providers. You will pay more for out-of-network providers and facilities.

UHC Choice Plus directory >



Flex and SAVE HDHP Plan
Use Flex Network’s Tier 1 and Tier 2. You will pay more for Tier 2 and out-of-network providers and facilities.

Note: The Tier 2 directory is the entire UHC Choice Plus directory. BSMH excludes some competing health systems/providers from Tier 2, and they are treated out-of-network or completely excluded (see the listings below). To verify Tier 2 providers, contact Rightway at 833-689-0542.

If an in-network provider or facility does not provide a complex service you may need, you may use the Network Exception Waiver to request a network exception before you receive care. If the out-of-network provider or facility is approved, your care will be covered under the Tier 2 network level or the in-network level.
